Неужели мы действительно преувеличиваем значение грудного вскармливания? Я так не думаю, и вот почему.

В интересном и провокационном исследовании, недавно опубликованном журнале Social Science and Medicine, говорится о преувеличении пользы грудного вскармливания. При умно выбранном экспериментальном дизайне авторы исследования проанализировали информацию из национальной базы данных Бюро статистики занятости населения при Министерстве труда США. Они поставили под сомнение факт влияния грудного вскармливания в семьях с маленькими детьми (больше одного ребенка) на следующие параметры из этой базы данных: индекс массы тела, ожирение, астма, гиперактивность, привязанность к родителям, послушание, осознание прочитанного, услышанного, понимание речи, математические способности, интеллект, основанный на памяти, обучаемость. Сначала исследователи обработали данные о всех семьях, в которых растут родные сестры и братья, и отметили, что дети, вскормленные грудью, развиты лучше, чем их братья или сестры, которых кормили смесью. Это касалось всех параметров кроме одного: к всеобщему удивлению, астма чаще встречалась у грудничков по сравнению с их родными братьями или сестрами - искусственниками. Когда исследователи тщательно изучили данные о семьях, где один ребенок был вскормлен грудью, а другой – искусственным питанием (такое углубленное изучение помогает контролировать другие способы вскармливания помимо грудного, влиянием которых можно объяснить существующие различия), они не обнаружили различий между братьями и сестрами ни по одному исследуемому параметру. Поэтому авторы сделали вывод о том, что значение и польза грудного вскармливания преувеличена.
 

В медиа и блогосфере эта информация приобрела статус сенсации, появились кричащие заголовки типа: Исследование утверждает: "Грудное молоко не лучше смеси". Преимущества грудного вскармливания завышены, переоценены или это самое лучшее для здоровья? Не спешите винить себя! Новое исследование показало, что польза грудного вскармливания сильно завышена.

Неужели эта публикация действительно подтверждает мнение, что грудное вскармливание не имеет той ценности, как принято считать? У меня есть несколько аргументов относительно этого мнения:

Во-первых, "неожиданные" результаты по астме – на самом деле не такие уж неожиданные. В минувшие десятилетия сформировалось убеждение в том, что грудное вскармливание предупреждает риск развития астмы. С тех пор как астма все чаще проявляется как наследственное заболевание, весьма вероятно, что матери, страдающие астмой или имеющие случаи заболевания астмой среди родственников, вынуждены делать все возможное, чтобы предотвратить развитие астмы у своих детей. И кажется весьма вероятным, что матери, участвующие в исследовании, страдающие астмой и/или имеющие случаи заболевания астмой в семье, были больше мотивированы на грудное вскармливание в попытке предупредить риск развития этого заболевания у своих детей, именно благодаря широко распространенному мнению о том, что грудное вскармливание защищает от астмы. А так как эти дети генетически более предрасположены к развитию астмы, у многих из них началась астма, несмотря на грудное вскармливание. В таком случае, грудное вскармливание здесь ни при чем, оно не увеличило вероятность развития астмы, наоборот, вероятность развития астмы у ребенка в связи с генетической предрасположенностью увеличивает возможный выбор матери в пользу кормления грудью своего ребенка. Таким образом, проведенный анализ установил связь, а модель исследования лишь выявила корреляцию между этими событиями, но не саму причину или направление причинно-следственной связи.

Во-вторых, авторы исследования рассматривали лишь влияние грудного вскармливания на детей. Но они совершенно не принимали во внимание значение и благотворное влияние грудного вскармливания на мать, например, защита от рака, диабета, сердечно-сосудистых и психических заболеваний.
Наконец, изучаемые факторы, выбранные авторами, кажутся мне весьма странными. Таблица, приведенная ниже, демонстрирует плюсы грудного вскармливания и риски от использования искусственного питания, опубликованные во многих научных изданиях:

Из списка выше я бы выбрал астму, ожирение и нейрокогнитивное развитие, как факторы, имеющие слабую связь между риском их развития и защитными свойствами грудного вскармливания. Другими словами, проведенное исследование не способно доказать с большой вероятностью, что грудное вскармливание уменьшает число случаев заболевания астмой, улучшает развитие мозга или защищает от ожирения. Для продвижения грудного вскармливания есть немало веских доводов общественного здравоохранения, помимо упомянутых в исследовании. И, как показано в таблице, их гораздо больше, чем эти три исследуемых фактора. Удивляет и то, что авторы выбрали именно ожирение, астму и несколько нейрокогнитивных показателей для изучения в своем исследовании. Эффект был бы таким же, если бы авторы изначально были настроены на отрицательные результаты исхода исследования, выбирая исследование факторов, которые в наименьшей степени демонстрируют какую-либо разницу между грудным вскармливанием и искусственным питанием. 

Итак, можно ли считать, что исследование наглядно демонстрирует то, что кормление смесью ничуть не уступает грудному вскармливанию? Я так не считаю. Доказало ли это исследование, что преимущества грудного вскармливания преувеличены? В отношении ожирения, астмы, и познавательного развития – возможно. Но в отношении многих других благотворных воздействий, которые грудное вскармливание оказывает на ребенка и мать – нет.
